Registrant Criteria: The Growth Code Conference For Pediatric Private Practices is designed for registrants that are solely owners or managers or employees of an Occupational Therapy, Physical Therapy, or Speech Therapy private practice. This is an exclusive conference hosted and co-sponsored by Fly High Business Builders, LLC DBA Wellness Works Management Partners. Any registrant that is classified as a consultant or vendor that is not a pre-approved sponsor, would not be eligible to attend The Growth Code Conference For Pediatric Private Practices. In the event that you fall under a vendor, consultant, or additional conflict of interest category, you must get pre-approval directly from Brandon Seigel who can be reached at brandon@wellnessworksmp.com. Failure to get pre-approval can result in automatic cancellation of your registration with a 10% holdback penalty.
No CEUs / Certificates Of Completion Policy: The Growth Code Conference For Pediatric Private Practices is not a CEU approved event in any capacity. All registrants understand that they are registering for a non-CEU conference and certificates of completion may be given at the time of the session if elected by the organizer but are not guaranteed in any capacity. Fly High Business Builders LLC, DBA Wellness Works Management Partners and all parties managing the Growth Code Conference for Pediatric Private Practices are not responsible for offering any continuing education credits or complying with registrant requests for continuing education proof connected to this event. All registrants are registering for this event with no expectations or demands of credit toward continuing education in any capacity.
Fly High Business Builders LLC, DBA Wellness Works Management Partners and all parties managing the Growth Code Conference for Pediatric Private Practices may elect to offer certificates of completion in the session only and final certificated are to be completed by the registrant, but this does grant any form of continuing education credit. After a session is complete or after the conference, registrants will not be eligible to receive a certificate of completion. If any certificates of completion are given, it will be done during the session itself. Fly High Business Builders LLC, DBA Wellness Works Management Partners and all parties managing the Growth Code Conference for Pediatric Private Practices will not provide digital or print out certificates after the fact in any capacity. In the event that a registrant does not get a certificate of completion, the only thing available is the receipt of registration under the original transaction.
Conference Confirmation Policy: Registration for The Growth Code Conference For Pediatric Private Practices (January 30, 2025 to February 2, 2025) will be confirmed when full payment is received. If you do not receive a confirmation within forty-eight hours after your completed registration and payment are submitted, please email admin@growthcodeconference.com.
Conference Cancellation Policy / All Sales Are Final & Non-Refundable / No Credits: The Growth Code Conference For Pediatric Private Practices has a strict no refund policy on all purchases including registration to the conference or tickets to additional events. Regardless of circumstances, there will not be any refunds or credits, or future credits made available. All registrations are final and non-refundable. Any purchaser has the right to explore additional insurance coverage at an additional expense, but Fly High Business Builders LLC, DBA Wellness Works Management Partners and all parties managing the Growth Code Conference for Pediatric Private Practices will not refund or credit purchasers in any capacity.
Fly High Business Builders LLC, DBA Wellness Works Management Partners and all parties managing the Growth Code Conference for Pediatric Private Practices reserves the right to refuse or cancel a registration at any time with no liability other than refunding the applicable registration fee minus 10% in the event that the Registrant is deemed a conflict of interest to attend. Registrants must be 21 years of age to register for the conference.
Hotel changes and cancellations policy All hotel reservations at the negotiated rate will be made directly through the hotel reservation link and are non-refundable and non-transferable. Fly High Business Builders LLC, DBA Wellness Works Management Partners and all parties managing the Growth Code Conference for Pediatric Private Practices hold no liability or responsibility related to your hotel reservation, cancellations, or refunds, etc. The hotel reservation is an agreement between the party that books the room and the hotel. The room block discount is extremely limited and the Growth Code Conference for Pediatric Private Practices has no bearing once discounted rooms are no longer available. For all matters related to hotel reservations, please contact the hotel directly. Badges: Badges are to be picked up at the event check-in area during the event. You must show a government issued picture ID to be issued your badge. Badges will not be issued without an ID and you will not be admitted into the venue without a badge.
By registering for the event, you agree that Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference for Pediatric Private Practices may use your email address and mobile phone number to send event updates, sponsorship promotions, and post-event survey evaluations. In addition, sponsors will be able to reach out to you based on your registration to our conference and participation.
Affiliated services
If the event website provides third party affiliated services information including accommodation information and transportation information or discounts, such information is provided solely as a convenience to you. Under no circumstances will Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference For Pediatric Private Practices be liable in any way for any third-party services, including, but not limited to, quality, availability, errors or omissions, or damages of any kind. You must evaluate and bear all risks associated with the use of any third-party services, including any reliance on the availability, quality, or usefulness of such services. You may be subject to additional terms and conditions that may apply when you use affiliated services.
Unforeseen circumstances
The event program schedule is subject to change at any time without prior notice.
Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference cannot guarantee the attendance of any exhibitor or speaker, or that any scheduled event will take place. Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference will not be held liable and no reimbursement will be made in the event of the cancellation or modification of the times, dates, sessions and/or events of an event as a result of an event that is beyond the Growth Code Conference and associated parties’ control, including but not limited to inclement weather, earthquake, flood, or other act of God, war, invasion, act of foreign enemy, hostilities, riot, national state of emergency, plague, pandemic, act/threat of terrorism, civil unrest, strikes or other labor disputes, or any other similar or related event which delays or prevents any or all of the event.
Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference for Pediatric Private Practices shall not be liable for damages of any kind resulting from errors or omissions in its conference materials, nor shall Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference for Pediatric Private Practices be liable for any damages resulting from the use of its conference materials or other information conveyed at the conference.
Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference for Pediatric Private Practices (company), and Hotel Colonnade, Coral Gables and adjacent participating hotels (hotel) will not be responsible for any personal injury or property damage arising from or in connection with attendance at the conference or participation in any conference activities or events.
Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference (company), and Hotel Colonnade, Coral Gables, and adjacent participating hotels (hotel) assume no responsibility for any losses due to theft or loss of equipment and articles or other personal property.
Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference (company), and Hotel Colonnade, Coral Gables and adjacent participating hotels (hotel) also reserve the right at their sole discretion and with no additional liability to refuse entrance, immediately eject and terminate the participation of any person whose conduct is deemed objectionable, disorderly, or disruptive to Fly High Business Builders LLC., DBA Wellness Works Management Partners and/or others on behalf of the Growth Code Conference (company), or facility, or in violation of any law.
Waiver & Release Of Liability:
I understand that, in connection with participating in the 2025 Growth Code Conference For Pediatric Private Practices (“the Event”) sponsored by the (Fly High Business Builders LLC, DBA Wellness Works Management Partners) (“Sponsor”) and held at Holiday Inn & Suites Clearwater Beach, and adjacent hotels (“Venue”), I may be exposed to persons who, knowingly or unknowingly, are infected with the novel coronavirus and/or have contracted COVID-19 and/or another communicable disease. The waiver, release and other representations set forth herein are given in consideration for the Sponsor permitting me to participate in the Event.
1. Acceptance of Risk; Release; Indemnification. I am fully aware that there are a number of risks associated with me entering onto the Venue and participating in the Event during the COVID-19 pandemic, including without limitation: (a) I could contract COVID-19 or other diseases which could result in a serious medical condition requiring medical treatment in a hospital or which could possibly lead to death; and (b) I will be subject to normal risks associated with participating in the Event, such as physical injuries or even death or loss or damage to personal property, including without limitation, from slips or falls, food poisoning or allergic reaction to food served during the Event, physical or verbal altercations with Venue staff or other Event participants, or injury due to the nature of the Event. On behalf of myself and my heirs, successors and assigns, I knowingly and freely assume all such risks, both known and unknown, relating to my participation in the Event and being present at the Venue, and I hereby forever release, waive, relinquish and discharge the Sponsor and its officers, directors, managers, officials, trustees, agents, employees or other representatives, and their successors and assigns, including but not limited to Association Headquarters, Inc. (AH) and its respective owners, shareholders, officers, managers, officials, directors, trustees, agents, employees and representatives (collectively, the “Sponsor Representatives”), from any and all claims, demands, liabilities, rights, damages, expenses, and causes of action of whatever kind or nature, and other losses of any kind, whether known or unknown, foreseen or unforeseen, (collectively, “Damages”) as a result of me being present on the Venue property and participating at the Event as described above, including but not limited to those related to the above described personal injuries, death, disease or property losses, or any other loss, and including but not limited to claims based on the alleged negligence of any Sponsor Representative or any other person. I further promise not to sue the Sponsor or any Sponsor Representative and agree to indemnify and hold them harmless from any and all Damages resulting from my being present at the Venue and participating in the Event.
2. Affirmative Representations. I represent and warrant that I will not enter the Venue or participate in the Event if (a) I am aware, as of the date of the Event, that I have contracted and not completely recovered from COVID-19 or another communicable disease; (b) I am aware, as of the date of the Event, that I have been exposed within the prior two weeks to someone who had contracted or was subsequently determined to have contracted, COVID-19 or another communicable disease; (c) I have been tested for COVID-19 or another communicable disease but not yet received the results of such test; or (d) I have symptoms consistent with having contracted COVID-19 or another communicable disease. I also represent and warrant that, during the course of the Event, I will comply with all applicable regulations and guidelines regarding COVID-19 and any other communicable disease, including those imposed by the Venue, the Sponsor, and the Sponsor Representatives
